Video Summary
The webinar launched the first part of a three-part LUGPA Learn CME series on BPH, led by Dr. Jason Hafron with Drs. Tom Muller and Ravi Rajan. The main message was to “treat the mechanism, not just the symptoms.” The speakers emphasized common mistakes in BPH care, such as assuming LUTS always means obstruction, relying only on DRE or PSA for prostate sizing, and escalating medications without objective testing.

They reviewed a stepwise evaluation approach: clarify symptoms, use IPSS as a starting point, perform objective testing such as uroflow/PVR, consider UroCuff or urodynamics when appropriate, assess prostate size with imaging, and use cystoscopy when needed to define anatomy and guide therapy. They noted that IPSS is useful but often underutilized and does not reliably predict obstruction.

The second half focused on medical therapy. Alpha blockers remain first-line, 5-alpha reductase inhibitors reduce progression risk, tadalafil may be underused, and combination therapy can help selected patients. However, medical therapy often improves symptoms without fixing the underlying problem, so ongoing monitoring is essential. The session ended with a reminder that proper evaluation should drive treatment decisions and that more webinar sessions would follow.
